The DFA is located in Pacific Mall, on your way to the Marcelo Fernan Bridge going towards Mactan. We have a dedicated liaison officer who can assist your queries and vet through your documents for application.
Please be reminded however, that as a travel agency, we do not guarantee approval or fast-tracking of your passport application and passport renewal. Approval is solely the prerogative of the DFA and on the strength of the documents you submit.

BASIC REQUIREMENTS IN GETTING YOUR PHILIPPINE PASSPORT
(Please prepare the original documents and one photocopy)
Basic Requirements for a First Time Applicant for Philippine Passport
- NSO – issued Birth Certificate. Please note that an authenticated Local Civil Registry – issued Birth Certificate may be suppletorily required if the entries in your Birth Certificate are illegible (unclear or cannot be read)
- NSO – issued Marriage Certificate (for married female applicants). Please note that an authenticated Local Civil Registry-issued Marriage Certificate may be suppletorily required if the entries in your Marriage Certificate are illegible (unclear or cannot be read)
- Any there (3) of the followings supporting documents:
- Voter’s ID
- NSO Marriage Contract
- Land Title
- Seaman’s Book
- Elementary of High School Form 137 or Transcript of Records with readable dry seal
- Government Service Record
- NBI Clearance
- Police Clearance
- Barangay Clearance
- Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant such as the following:
- NSO Birth Certificate of child/children
- SSS – E1 Form or Microfilmed Copy of SSS – E1 Form
- Income Tax Return
- Voter’s Certification, List of Voter’s Registration Record (for newly issued VRR, please attach receipt)
- School Yearbook
- Community Tax Certificate
- At least one (1) valid Identification Card
- Government-issued picture IDs such as the following:
- Digitized SSS ID
- Driver’s License
- GSIS E-card
- PRC ID
- IBP ID
- OWWA ID
- Digitized BIR ID
- Senior Citizen’s ID
- Other acceptable picture ID’s such as the following:
- Old College ID
- Alumni ID
- Old Employment IDs
- Current Employment ID with Employment Certificate
- Government-issued picture IDs such as the following:
- Duly accomplished passport application form
First Time Application and Renewal of Philippine Passport of a MINOR
- NSO-issued Birth Certificate
- NSO-issued Marriage Contract of the Child’s parents, if applicable
- Baptismal Certificate
- Original passport of the minor’s mother (for illegitimate children) or original passport of either parent (for legitimate children)
- School ID, if the minor is a student
- Original Passport of the minor (if the application is for renewal)
Requirements for Philippine Passport Renewal
- Original passport
- Photocopy of the date page and back page of the passport
- At least two (2) valid identification cards
- If your current passport does not show the city of your birth place, please
- Submit your NSO-issued birth certificate or present your previous green passport, for substantiation purposes
Requirements for Lost Valid or Unexpired Philippine Passport
- All basic requirements for a first-time application
- Affidavit of Loss
- Police Report
- Clearing period of fifteen (15) working days
- Penalty Charge of P200 (P350 for a lost electronic passport)
- Copy of passport that was lost, if one was kept

IMPORTANT
Please note that depending on the completeness and nature of your application, questions may be asked for additional documents may be required by the Passport Officer. In all cases, personal appearance of the application is mandatory.
